SHIEC – Strategic Health Information Exchange Collaborative

California – SacValley MedShare

SacValley MedShare (@svmshie) is a member of the California Association of Health Information Exchanges (CAHIE) and serves the 16-county region of California, north of Sacramento, which includes the North Central Valley and the surrounding mountainous terrain. Its region is largely rural and mountainous, with travel limited by climate and season in much of the area. Good communication is critical to coordinate care between primary care, specialists, hospital providers, and skilled nursing care facilities that fulfill a vital role in post-discharge care for many patients.

SacValley MedShare and Connect Healthcare Form Largest and Fastest Growing Health Information Exchange in California – Doctors and medical technicians will be able to quickly and securely access the medical records of 2.6 million patients in Northern California following the formation of what becomes the geographically largest and fastest growing HIE in the state. SacValley MedShare, a non-profit HIE based out of Northern California, has agreed to merge with Connect Healthcare, a non-profit HIE covering the counties of Sonoma, Napa, Solano and Yolo. The resulting HIE, which will retain the SacValley MedShare name, will provide health information exchange services to hospitals, clinics and physicians in 16 counties across 37,799 square miles, stretching from the Sacramento Valley to the Oregon and Nevada borders.

HealthSYNC of Louisiana

HealthSYNC of Louisiana (@HealthSYNCofLA) is a physician-led health information network delivered in partnership with the Louisiana State Medical Society (LSMS) offering a suite of health information technology tools to help health professionals across the state of Louisiana CONNECT. ANALYZE. ENGAGE. and TRANSFORM. the healthcare industry.

healtheConnect Alaska

healtheConnect Alaska (@akehealth) is a 501(c)(3) non-profit corporation, organized and managed by Alaskans and tasked by the State of Alaska’s Department of Health and Social Services, to provide safe, secure transport for health information in order to improve quality and safety of patient care and increase efficiencies for hospitals and medical practices. healtheConnect Alaska provides health information exchange and direct secure messaging services to more than 20 hospitals and 450 healthcare organizations across Alaska.

State’s health information exchange announces rebrand and renewed commitment to providing value and long-term sustainability – The Alaska e-health Network, the non-profit tasked with establishing and maintaining Alaska’s secure statewide health information exchange, announced a new brand identity, along with new initiatives, strategic technology partnerships, and a renewed commitment to linking all Alaska healthcare providers to a robust, responsive, and secure statewide network for sharing electronic health information.

Maine – HealthInfoNet

HealthInfoNet (@hinfonet) is a Maine-based, independent nonprofit designated by the State of Maine to operate Maine’s statewide health information exchange. The HIE links medical information from separate health care sites to create a single electronic patient health record. This record is accessible to a patient’s authorized health care providers to support treatment. HealthInfoNet provides a secure, standardized electronic system where health care providers can share important patient information, giving them the tools they need to make more informed treatment decisions, especially in an emergency.

Use Case Report: Central Maine HealthCare – In early 2017, Central Maine Health Care (CMHC) Accountable Care Organization began to use HealthInfonNet’s Analytics and Reporting Platform (HARP). CMHC’s implementation of HARP replaced time-consuming manual processes resulting in time-saving efficiencies, staff satisfaction and improved patient outcomes. The CMHC Use Report highlights workflows, documentation, adoption, and outcomes.

PA-HSX-200Health Share Exchange of Southeastern Pennsylvania (HSX)

HealthShare Exchange (@HealthShareExch) is the greater Philadelphia region’s health information exchange for the five-county Southeastern Pennsylvania region. HSX is a unique, membership-dues-supported nonprofit formed by the Delaware Valley’s hospitals/health systems and healthcare insurers that links the electronic medical record systems of different hospital health systems, medical centers, and clinics — and the claims data of healthcare insurers — to make this information accessible at inpatient and outpatient points of care, including medical practice offices.

HealthShare Exchange Selects Onpoint as CPC+ Partner – Onpoint is pleased to partner with HealthShare Exchange (HSX) as their data aggregation and reporting contractor for the Greater Philadelphia region’s Comprehensive Primary Care Plus (CPC+) initiative. HealthShare Exchange, established in 2012, is the health information exchange (HIE) for Greater Philadelphia and Delaware Valley, including southeastern Pennsylvania and southern New Jersey. Their goals include the reduction of unnecessary care and readmissions through electronic sharing of medical records and claims data across their service area.

Wisconsin Statewide Health Information Network (WISHIN)

wi-shin-twitterIn October 2010 Wisconsin officially announced its intention to designate WISHIN (@WISHIN_Pulse), a not-for-profit organization formed by the Wisconsin Hospital Association, Wisconsin Medical Society, Wisconsin Collaborative for Healthcare Quality, and Wisconsin Health Information Organization, as the State-designated entity to govern statewide HIE. DE on October 25, 2010. WISHIN is responsible for implementing Wisconsin’s Strategic and Operational Plan. The plan requires multi-stakeholder collaboration and emphasizes ongoing development of government and policy structures.

Delaware Health Information Network (DHIN)

DHIN-twitterDelaware Health Information Network’s (@DHIN_HIE) roots reach back to 1997 when the Delaware General Assembly enacted the organization as a public/private partnership. The objective was, and remains today, to advance the creation of a statewide health information network that addresses the state’s needs for timely, reliable and relevant health care information. In 2007, DHIN “went live” – meaning it became the first operational statewide health information exchange in the nation.

DHIN Docks in Lewes for Summer Health IT Summit – Nearly 75 industry leaders from across the region and country joined Delaware Health Information Network for a Healthcare Leadership Summit on June 14th in Lewes. Lieutenant Governor Bethany Hall-Long and Delaware Health & Social Services Secretary Dr. Kara Odom Walker were among the presenters at the day-long retreat at University of Delaware’s Virden Center.

New York -Statewide Health Information Network for New York (SHIN-NY)

NY-SHINY-200The SHIN-NY is the only HIE that provides universal access regardless of network or EHR system. New York State has made a significant investment in the SHIN-NY and in 2016 they set a goal of connecting 100% of the PPS across the state to the SHIN-NY. The eight RHIOs that make up the SHIN-NY are working directly with the PPS organizations to meet their needs.

Colorado Regional Health Information Exchange (CORHIO)

Utah Health Information Network (UHIN)

UT-UHIN-200UHIN (@UHIN_HIE) has been in operation since 1993 beginning as a claims “post office.” Today it also operates the Clinical Health Information Exchange (cHIE), which empowers doctors and patients to work together for safer, better-coordinated care by making crucial information available to doctors at the point of care.

UHIN Connects to 8 New States – UHIN is excited to announce that it is now connected to health information exchanges (HIEs) in 8 additional states through its new connections to the Heartland and Central implementations of the Strategic Health Information Exchange Collaborative’s (SHIEC) Patient Centered Data Home (PCDH) initiative. The PCDH initiative ensures that vital healthcare information follows patients no matter where care is delivered. PCDH enables primary care physicians in Utah subscribing to admission/discharge/transfer (ADT) notifications for their patients to be alerted when their patient has a medical encounter in one of the newly-added HIEs’ jurisdictions.

Texas – Healthcare Access San Antonio (HASA)

Greater Houston HealthConnect

TX-HealthConnect-twitterFormerly the Greater Houston Health Information Exchange and now renamed Greater Houston Healthconnect (@HealthconnectTX) is launching one of the largest health information networks in the country. Healthconnect is working collaboratively with the Texas Health Services Authority and the Texas HIE Coalition to provide connectivity throughout the state and to share value-added services.

HCA Healthcare’s Gulf Coast Division – the Region’s Largest Health System – Joins Greater Houston Healthconnect – HCA Healthcare’s Gulf Coast Division has joined Greater Houston Healthconnect, the region’s Health Information Exchange (HIE). The Gulf Coast Division’s participation represents a major milestone for the community, as it is the final major health system to join the HIE. As the largest health system in Southeast Texas, HCA Healthcare’s Gulf Coast Division community footprint is robust, with 17 acute care hospitals, emergency centers, nine ambulatory surgery centers, more than 5,000 active physicians, and over 16,500 employees.
