2018 State Healthcare IT Connect Summit

Save the date! The 2018 Meeting is the 9th Annual State Healthcare IT Connect Summit bringing together public and private sector thought leaders to share ideas and benchmark implementation strategies of State health IT systems as they move forward with diverse health and human services transformation programs. The meeting comprises keynotes, keynote panels, collaborative networking roundtables (Meet the Innovators and States of Implementation), the Connect Exhibition and breakouts assigned to 5 separate tracks. Connecticut – CTHealthLink

CTHealthLink (@CTHealthLink) is a physician-led health information network delivered in partnership with the Connecticut State Medical Society (CSMS) offering a suite of health information technology tools to help health professionals across the state of Connecticut CONNECT. ANALYZE. ENGAGE. and TRANSFORM. the healthcare industry.

CMS Announces CTHealthLink – In support of its mission to improve the quality of care and safeguard the health of patients, the Connecticut State Medical Society is pleased to announce CTHealthLink, a statewide health information exchange (HIE) that is now live. CTHealthLink encompasses an interoperable health information exchange, as well as patient health portals, extensive data analytics tools, and population health solutions.

Nebraska Health Information Initiative (NeHII)

Nebraska Health Information Initiative (@NeHIIorg) is leading the effort to create a secure, online, web-based Health Information Exchange designed to serve the state of Nebraska. Improved delivery of health care information and reduced health care costs are good for all citizens of Nebraska. The state has a proud tradition and reputation of providing the finest health care possible to its citizens. The HIE will help health care providers take health care to the next level.

Nebraska HIE implements improved prescription monitoring – To enhance the state’s Prescription Drug Monitoring Program, the Nebraska Health Information Initiative and the Nebraska Department of Health and Human Services will use electronic prescribing and medication management solutions from vendor DrFirst. The goal is to capture state prescription information for the program to improve patient safety and optimize health outcomes.

Maine – HealthInfoNet

HealthInfoNet (@hinfonet) is a Maine-based, independent nonprofit designated by the State of Maine to operate Maine’s statewide health information exchange. The HIE links medical information from separate health care sites to create a single electronic patient health record. This record is accessible to a patient’s authorized health care providers to support treatment. HealthInfoNet provides a secure, standardized electronic system where health care providers can share important patient information, giving them the tools they need to make more informed treatment decisions, especially in an emergency.

Webinar: Leveraging User-Centered Technology to Improve Health Outcomes – This All In project showcase webinar featured presentations from two project teams, HealthInfoNet and Essential Access Health, that are designing and implementing user-centered, interactive applications that leverage data from EHRs and other sources to improve the health of under-resourced populations. All In: Data for Community Health is a nationwide learning collaborative that helps communities build capacity to address the social determinants of health through multi-sector data sharing collaborations.

Texas – Paso del Norte Health Information Exchange (PHIX)

The Paso del Norte Health Information Exchange (@PHIXNetwork) is a unique collaborative of major health care leaders in El Paso. PHIX pulls health information from the EHRs of all providers in the PHIX network and stores it in a centralized record so that patients’ health information in one place. PHIX provides HIPAA- compliant email and supports physician practices in optimizing their utilization of EHR technology

Paso del Norte Health Information Exchange Achieves Highest Level of Data
Security Accreditation – The Paso del Norte Health Information Exchange recently announced it has achieved full accreditation with the Texas Health Information Exchange Accreditation Program from the Electronic Healthcare Network Accreditation Commission and the Texas Health Services Authority. Texas HIE Accreditation ensures HIEs within Texas are interoperable with state and federal programs, and provide the private, secure and proper exchange of health information in accordance with established laws and public policy.

 

PA-HSX-200Health Share Exchange of Southeastern Pennsylvania (HSX)

HealthShare Exchange (@HealthShareExch) is the greater Philadelphia region’s health information exchange for the five-county Southeastern Pennsylvania region. HSX is a unique, membership-dues-supported nonprofit formed by the Delaware Valley’s hospitals/health systems and healthcare insurers that links the electronic medical record systems of different hospital health systems, medical centers, and clinics — and the claims data of healthcare insurers — to make this information accessible at inpatient and outpatient points of care, including medical practice offices.

A Day in the Green for HSX’ers, All for a Good Cause — Neighborhood Food – On a warm summery Thursday, HSX staff members got their hands dirty weeding, harvesting, tilling, and planting in their third annual community service day.

Wisconsin Statewide Health Information Network (WISHIN)

wi-shin-twitterIn October 2010 Wisconsin officially announced its intention to designate WISHIN (@WISHIN_Pulse), a not-for-profit organization formed by the Wisconsin Hospital Association, Wisconsin Medical Society, Wisconsin Collaborative for Healthcare Quality, and Wisconsin Health Information Organization, as the State-designated entity to govern statewide HIE. DE on October 25, 2010. WISHIN is responsible for implementing Wisconsin’s Strategic and Operational Plan. The plan requires multi-stakeholder collaboration and emphasizes ongoing development of government and policy structures.

Who Can Use WISHIN Pulse? – As they are assisting clients in the onboarding process, WISHIN implementation specialists are frequently asked, “Who on my staff is allowed to access WISHIN?” The short and simple answer is: anyone who has access to patient health records and is involved in providing care to patients or supporting provision of care. NY-SHINY-200New York -Statewide Health Information Network for New York (SHIN-NY)

The SHIN-NY is the only HIE that provides universal access regardless of network or EHR system. New York State has made a significant investment in the SHIN-NY and in 2016 they set a goal of connecting 100% of the PPS across the state to the SHIN-NY. The eight RHIOs that make up the SHIN-NY are working directly with the PPS organizations to meet their needs.

Healthix HIE Connects with Mount Sinai, Northwell Exchanges to Share Data – Healthix enables the sharing of data from 16 million patients in the New York City metropolitan area and Long Island and is now connected with two private HIEs serving those population areas. The integration means that clinical data generated by Northwell and Mount Sinai is available in real time to every other organization in the Healthix network, and vice versa, without requiring the use of multiple interfaces, Healthix said.

Colorado Regional Health Information Exchange (CORHIO)

CORHIO200CORHIO (@CORHIO) manages one of the country’s largest and most successful health information exchange networks and works to improve healthcare through the use of information technology and data exchange.

Hundreds of Providers and Staff at One of the Nation’s Largest FQHCs Now Live on CORHIO – With more than 90,000 patients cared for in 26 outpatient centers, Peak Vista Community Health Centers is one of the nation’s largest Federally Qualified Health Centers. Many of Peak Vista’s healthcare providers and support staff recently gained access to CORHIO’s PatientCare 360, which will help improve clinical decision-making and care coordination for patients in the Pikes Peak and East Central regions. Michigan – Great Lakes Health Connect (GLHC)

GLHC twitterGreat Lakes Health Connect (@GLHC_HIE) is one of the largest and most successful Health Information Exchanges in the country and is growing rapidly. GLHC is committed to creating care-connected communities across Michigan. Their vision is to enhance patient experience, improve health outcomes, and reduce costs across the continuum of care.

Patient Information from Five States now available in Michigan through Great Lakes Health Connect Community Health Record – Out-of-state patient information is now available in real-time through Great Lakes Health Connect. This health data exchange expands interoperability and reduces barriers to care for providers and residents in five Midwest states. GLHC is the first HIE within the recently launched Heartland Project to connect to all of the other health information exchange organizations involved in the initiative.

Utah Health Information Network (UHIN)

UT-UHIN-200UHIN (#UHIN) has been in operation since 1993 beginning as a claims “post office.” Today it also operates the Clinical Health Information Exchange (cHIE), which empowers doctors and patients to work together for safer, better-coordinated care by making crucial information available to doctors at the point of care.

UHIN Connects with Nebraska Health Information Initiative – UHIN is excited to announce that it is now connected with the Nebraska Health Information Initiative! As part of the multi-state Patient Centered Data Home, Admit / Discharge / Transfer notifications will be sent to those participating Utah providers for their patients experiencing a medical encounter in Nebraska. Conversely ADT notifications for Nebraskans receiving medical care in Utah also will be sent to the primary care physician back home.

Indiana Health Information Exchange (IHIE)

IHIE-200Indiana Health Information Exchange (@IndianaHIE) operates the nation’s largest health information exchange, providing a secure, robust and interoperable health information technology network that connects over 150 hospitals, long-term care facilities, rehabilitation centers, community health clinics and other healthcare providers.

Top Ten Reasons to Work at IHIE -The Indiana Health Information Exchange was recently awarded a 2017 Top Workplaces honor by The Indianapolis Star just weeks after being named to Modern Healthcare’s Best Places to Work in Healthcare list for the third consecutive year.
